Pyramids scored a stoppage-time equaliser, ending Mamelodi Sundowns' celebration. The African Champions League final is now tied at 1-1 after the first leg in Pretoria.
The match took place at a sold-out Loftus Versfeld. Sundowns took the lead eight minutes into the second half. Forward Lucas Ribeiro followed up a shot and scored from close range.
Sundowns fans were excited, thinking they had secured an important win. However, Walid El Karti headed in a cross from Mohanad Lasheen in the 94th minute. His goal went past Sundowns keeper Ronwen Williams.
With the away goals rule still in effect, this late goal could help Pyramids. They are first-time finalists and could win $4 million in prize money.
Founded in 2008, Pyramids has become one of Egypt's top teams. This transformation happened after Saudi Arabian investors took over in 2018.
Sundowns are aiming for their second continental title. They last reached the final in 2016 and will play in FIFA's expanded Club World Cup next month.
The two teams will meet again on Sunday, June 1, at the 30 June Stadium in Cairo at 17:00 GMT.
